The Residence | Long-Term Skilled Nursing Care
At Saint Antoine Residence, we provide individualized 24-hour skilled care and rehabilitation services in a warm and family-friendly environment. Our long-term care facility provides nursing support and assistance with all activities of daily living [ADLs], including dressing, eating, attending to hygiene, toileting, and walking (or functional mobility).
Our interdisciplinary team takes a holistic approach to the patient’s care plan development. Members of The Residence Interdisciplinary Care Team include representation from the following departments: Nursing, Case Management, Social Worker, Physical Therapist, Occupational Therapist, Dietitian, and Activities.
Our dedicated team works closely with the patients and their family members to determine the optimal treatment plan for each individual situation.
When is it time to consider long-term care?
- My loved one requires total assistance with daily tasks such as bathing, dressing, grooming, and toileting.
- My loved one has a chronic condition such as diabetes, CHF, COPD,
- My loved one has a neurological condition, such as Parkinson’s, and they requires around-the-clock skilled care.
- My loved one has a history of falls.
- My loved one requires insulin management, assistance with nebulizers/inhalers, treatment for wound care, etc
- My loved one requires medication management for prescriptions and/or over-the-counter medications.
- My loved one is not self-motivated and relies on others for reminders and/or guidance through the activities of their day
- My loved one would benefit from a personalized care plan designed by a team of medical professionals
